Child Custody Forms

Child custody forms are used to petition the court for custody, partial custody, and/or visitation rights. There are also versions available that are designed specifically for modifying an existing custody order or to advise the court that an existing order is being violated. All of these forms can be submitted without the assistance of an attorney if a parent wishes to represent him/herself. Court fees still apply in such cases. Grandparents and other relatives may also use such forms to request visitation or custody. Filling out each form fully and correctly is imperative. A typical child custody form may include such information as the names and addresses of all persons involved (plaintiff, defendant, and minor child). Information about both biological parents and any other people who may have custody or visitation rights is also required so they can be notified of the petition. The person filing the form must also state a reason that the best interests of the child would be served by granting custody or visitation rights to the plaintiff.
