Paternity Forms

There are many uniform laws surrounding paternity issues, and this allows for some standard names to be applied to the paternity forms necessary in many legal settings. For example, paternity forms might include a complaint called a Contested Paternity Action, or an acknowledgement form called a Voluntary Acknowledgement of Parentage (Paternity). Filling out and submitting such forms usually indicates that some sort of support is going to be sought out by a mother, or a man is rejecting claims that he is the father of a child. Why is paternity important? It doesn't just identify the biological parents of a child, but also allows for financial support, public assistance, legal identity and even benefits such as life insurance or veteran's coverage. All paternity forms are dealt with through the court system within the state where the custodial parent of the child resides.

Fast Facts

  • All states use different paternity forms, though they may have similar names and associated actions.
  • Even when a father's name is on the birth certificate, paternity can still be challenged using DNA testing.
